TITLE:  Physician

DEPARTMENT:   Adult Medicine/ Internal Medicine/ Family Practice

CLASSIFICATION: Contract / Salary

RESPONSIBLE TO:   Chief Medical Officer                

FUNCTION:

  • To provide comprehensive primary care, direct patient health care services,  to adults and/or minors above the age of 16 years
  • To provide direct and indirect patient health care services based on the Patient-Centered Medical Home (PCMH) model and medical evidence-based guidelines; 
  • To provide indirect health care services to the patients;
  • To complete all forms of medical documentation in electronic health records;
  • To work cooperatively with the health care team for the benefit of the patients;
  • To communicate fully with the patients and their families and/or caregiver;
  • To participate in all WinMed CCQI (Clinical Continuous Quality Improvement) activities pertinent to patients over the age of 16 years;
  • To adhere to and follow the Policies and Procedures of WinMed Health Services   

 

DUTIES AND RESPONSIBILITIES:

To provide comprehensive primary care, direct patient health care services, to adults and/or minors above the age of 16 years:  

  1. The physician will see patients and provide comprehensive primary care services to them as per the agreed upon work schedule.
  2. The physician will review the notes of the intake/ medical assistant/ nurse regarding that patient as well as the past medical history and history of past visits.
  3. The physician will perform his/her office visit with the patient, including history, physical, assessment, treatment plan, and follow-up required.
  4. The physician will perform the needed medical record documentation consistent with the standards of the “SOAP” and health maintenance.

 

To provide direct and indirect patient health care services based on the Patient-Centered Medical Home (PCMH) model and medical evidence-based guidelines:

  1. The physician will provide direct and indirect patient health care services based on Patient-Centered Medical Home (PCMH) model of health care and evidence-based guidelines.
  2. In addition to following health maintenance guidelines and age-based protocols, the physician will educate and actively promote the patient  to become more involved with his/her health self-management.
  3. The physician will subscribe to and promote health outcomes consistent with the standards of his/her profession and well as those espoused by WinMed’s CCQI activities.

 

To provide indirect health care services to the patients:

  1. The physician will perform indirect patient care services as needed, when needed.
  2. These indirect patient care services may include but are not restricted to:
    1. Review and sign-off of labs results within 24-48 hours of return
    2. Review and sign-off of test results within 24-48 hours of return
    3. Complete referrals within 24-48 hours of decision to refer a patient
    4. Review referral and consultation results within 2-3 days of receipt
    5. Communicate all results to patients in the most appropriate manner: letter, office visit, phone call, etc.
    6. Complete duties with or delegated to clinical support staff: forms, DME requests, Home Health care orders, FMLA forms, Energy Assistance forms, and any others that patients may need help with.
  3. If these duties cannot be discharged within the time frame, they will be signed out to a colleague or staff member to complete, such as during a physician’s absence or time off.
  4. If indirect health care services require an “inordinate “ amount of time, the physician may choose to convert this task to a direct patient care services and request a patient’s office visit.

 

To complete all forms of medical documentation in electronic health records: WinMed is using electronic health records (EHR) to record and store medical documentation for patients.

  1. The physician is expected to complete all medical record documentation in the electronic health record.
  2. These may include but are not restricted to:
    1. Office visit documentation
    2. Telephone messages
    3. On-Call telephone messages
    4. Sign-offs of lab, tests, and other results and correspondences
    5. Task duties to other staff
    6. Review and sign-off in Provider Approved Queue (PAQ)

 

To work cooperatively with the health care team for the benefit of the patients:

  1. The physician is to interact with the health care team in a professional manner at all times.
  2. The physician is assisted in talking care of patients by the clinical support and non-clinical support staffs.
  3. The physician serves the patients best when he/she works cooperatively with the staff.
  4. The physician may assign duties to the staff consistent with their level of training and certification/ licensure. These duties may include but are not restricted to completion of paperwork, calling in non-narcotic refills, and sending letters to patients on results or requests for appointments.
  5. If there are any problems with the staff, the physician is to inform the Chief Medical Officer or Supervisor who will address the issue with the staff’s supervisor.

 

To communicate fully with the patients and their families and/or caregiver:

  1. The physician must be prepared to communicate effectively with patients, their families and/ or caregivers.
  2. The physician is expected to promote and discuss self-management goals, provide community support resources information, and other aids/ assistance that will support and improve the patients’ health status outcomes.
  3. Every communication with the patients must be documented in the medical record.

 

To participate in all WinMed CCQI (Clinical Continuous Quality Improvement) activities pertinent to patients over the age of 16 years:

  1. The physician will participate in all WinMed CCQI activities for his/ her patients over the age of 16 years.
  2. These activities include but are not restricted to:
    1. Chronic Disease Care management
    2. PCMH recognition and accreditation
    3. Meaningful Use Attestation and compliance
    4. Infection Control
    5. EHR
    6. Peer Review
    7. Risk Management and Prevention
    8. Completion of Accident/ Incident Forms for any potentially legally-liable events  
  3. The physician will participate in meetings required by WinMed unless excused by the Chief Medical Officer. These meetings include but are not restricted to:
    1. Office Staff Meetings
    2. Provider Meetings
    3. Site Team Meetings
    4. Department Meetings
    5. Other meetings as deemed necessary

 

To adhere to and follow the Policies and Procedures of WinMed Health Services:  

  1. The physician will adhere to and follow the Policies and Procedures of WinMed Health Services.
  2. If the physician identifies an area that needs a new or revised policy or procedure due to an issue or change in the environment, he /she must notify the Chief Medical Officer so it can be addressed.

 

QUALIFICATIONS: Medical

  • Must be licensed in the State of Ohio and have a valid license at the time of application and hire
  • Must have a valid DEA number at the time of application and hire
  • Must have matriculated in an accredited Medical College or Osteopathic College and received a Medical Degree (MD or DO).  
  • Must have completed satisfactorily a post-medical degree training program in Internal Medicine, Internal Medicine-Pediatrics, or Family Practice.
  • Must be Board-Certified if out of training for more than 5 years or Board-Eligible if less than 5 years.
  • Experience with electronic health records preferred but not required.
  • Experience with continuous clinical quality improvement or collaboratives for chronic diseases and/or patient-centered medical home, preferred but not required
  • Must be current in CPR certification.
  • Must be able to be credentialed with insurance companies and hospitals.
  • Must be able to function in an emergency situation.
  • May be ACLS or Advanced Life Support accredited.

 

QUALIFICATIONS:  General

  • Must have moderate amount of computer skills or be trainable.
  • Must be able to adapt to changes within the environment i.e.: (professional, physical, and clinical).
  • Must be able to accept constructive criticism.
  • Must be flexible on meeting varied work assignments.
  • Must be able to deal compassionately with people from diverse backgrounds and socioeconomic conditions
